Observations have five different question types designed to fairly and accurately assess the user's performance and assign a compliance status. This article describes each question type and how they are scored. For each question, one is marked as the correct answer.

Designed to assess knowledge of compliance for critical tasks. These are often used for answers with minimal variation and offers three possible responses. |
- compliant (User receives credit) - non-compliant (User does not receive credit) - not applicable (User's credit is deducted) |
Partial Compliance |
Partially compliant refers to evaluating which aspects of the requirements are met, though not all are fulfilled. This question can assist with identifying training gaps and misconceptions. |

Safe / At Risk |
Designed to measure if Users understanding is of minimal to no potential for harm or danger (safe), or if Users understanding has significant possibility of negative consequences or injury (At risk). |
- Safe (User receives credit) - At Risk (User does not receive credit) - Not Applicable (The user's credit is deducted) |
Scaled Score |
Represents a user's response as a percentage instead of a whole. Scaled scores are effective indicators of a user's performance level on tasks, especially those that demand a high degree of thoroughness. |

Yes/No |
These are often used for answers in a closed-ended response instead of allowing for open-ended discussion since only two possible responses are available. |
